Unvented Hot Water Cylinder Issues

Below are some images in which faults with unvented hot water cylinders have occurred.
Please take a look closely and if you see anything similar with your hot water cylinder, please get in touch straight away!

Expansion Vessel (white tank) piped up incorrectly.

It shouldnt go into the side of the inlet and its not to manufacturers instructions.
Discharge pipe (red line) has an uphill flow. 

Not to manufacturers instructions.
Expansion Vessel (white tank) piped up incorrectly.

It shouldnt go into the top of the unvented hot water cylinder and its not to manufacturers instructions.
The TPRV (temp/pressure relief) valve had leaked for so long, that it had started to rot the casing. 
Expansion Vessel (red tank) is inaccessible and would be difficult to service.

Expansion Vessel (white tank) piped up incorrectly.

It shouldnt go into the top of the unvented hot water cylinder and its not to manufacturers instructions.
This is by far the most dangerous issue we came across!

The safety valve (PRV) was discharging water through the Tundish (black cup with hole in the middle) and a plumber added an isolation valve to it.

If the customer didnt call us as fast as they did, there would have been a major issue and potential explosion.
